Ovarian torsion is an uncommon cause of acute abdominal pain in pediatric patients. Presenting symptoms are nonspecific, requiring a high index of suspicion for early diagnosis, essential to increase the viability of the ovary. We report an 11-year-old, premenarcheal girl, who presented to the emergency department with 4 hours of vomiting and abdominal pain. Upon admission, she experienced initial partial improvement, after 48 hour abdominal pain worsened and fever appeared. Abdominal radiography was performed suggestive of an abdominal mass and abdominal ultrasound and MRI imaging suggestive of ovarian tumor. After surgery it was the final diagnosis of ovarian torsion. In this case we want to emphasize highly nonspecific clinical presentation of this entity, which together with additional test often inconclusive or misleading, make early diagnosis difficult without a high degree of suspicion.
