Although the available therapeutic tools are more and more numerous, in grade II and III obesity usually medical treatment is usually insufficient. Surgery is a promising option for those obesity patients in which conventional treatments have been ineffective. In selected patients, chirurgic treatment is very successful in weight loss and long-term maintenance, and above all, in the control of comorbidities associated with obesity. Moreover, overall mortality reduction has be demonstrated. Because the risk and complications of obesity surgery, patients have to be carefully selected by multidisciplinary team in specialized health-centers.
