Abstract

Introduction: Prostate cancer remains one of the most common solid neoplasms with significant morbidity and mortality in men. The incidence of testicular metastases from prostatic adenocarcinomas is very rare and is usually associated with high histological grades and advanced stages of the disease. Case report: A 47-year-old male presented with urinary irritative symptoms and elevated PSA levels. Imaging revealed a lesion in the right prostatic lobe and multiple variable-sized nodules in the left testicle, suspicious for a non-seminomatous tumor. Transrectal prostatic biopsy and left orchiectomy were performed, leading to a diagnosis of bilateral adenocarcinoma, ISUP grade 4, and testicular metastasis from prostatic adenocarcinoma. Discussion: Secondary testicular metastases are extremely rare; however, among them, prostatic carcinoma is the solid neoplasm most frequently metastasizing to this location. They may present as asymptomatic testicular lesions, often associated with significant elevations in serum PSA levels (>100). Typically diagnosed several years after the onset of the disease, their presence is associated with unfavorable prognosis.
