Abstract

Introduction: Non-painful testicular neoplasms diagnosed in young patients require radical treatment with orchiectomy for proper classification. The frequency of syphilitic orchitis in our environment is low, being a manifestation reported in only a few cases, typically as a manifestation of tertiary syphilis. Case Report: A 34-year-old man was diagnosed with suspected testicular neoplasm. His medical history included two previous instances of sexually transmitted disease, specifically syphilis. Histological examination revealed necrotizing granulomatous inflammation, suggestive of syphilitic gumma in the appropriate clinical context. Discussion: The progression of syphilis, analytical findings, treponemal tests, and radiological images are fundamental for establishing the diagnosis of syphilitic orchitis. Most cases reported in the literature have been treated with orchiectomy due to the suspicion of testicular neoplasm. However, the cases reported in the literature advocate for conservative treatment with antibiotics and follow-up with radiological imaging.
