Radioterapia intraoperatoria en el tratamiento del cáncer de cabeza y cuello localmente avanzado y/o recurrente: Experiencia inicial

Resumen
From April 1984 to June 1989, 17 patients with the established diagnosis of squeamous cell carcinoma of the head and neck have been treated with surgery and intraoperative radiotherapy (IORT) with or without external beam fractionated irradiation in a loco-regional intensification combined treatment program. Median age was 58 years (35-78 years), with a performance status up to 70% in 80% of the patients (Karnofsky scale). The tumour was localized in 9, locally advanced in 7, and metastasic in 1 patient. Primary tumour sites were: buccal mucosa, 4, parotid gland 2, larynix 2, tonque, hypopharynx, lip, ear, pyriform sinus, retromolar trigone and nasopharynx in 1 case. Seven local failures (40%) and 1 local and distant relapse was observed. Toxicity was mild and reversible. Median survival time for the entire group is 10 months. With a follow-up ranging from 5 to 55+ months, 7/17 patients (40%) are still alive without evidence of disease, intraoperative therapy is a feasiblel technique acceptably tolerated which promote local control tumour and could achieve some long term survivors specially in patients amenable to radical surgery whit microscopic post-surgical residual disease. The consideration of performing a miocutaneous flap in patients candidates to multidisciplinary treatment approach including IORT is recommended for locally advanced neck disease and/or skin involvement.
