Resumen
El síndrome de embolia grasa, mucho más frecuente que la embolia grasa, ocurre en varones entre 20 y 40 años con fractura de largos huesos. La presentación clínica característica incluye síntomas respiratorios, neurológicos y rash cutáneo petequial que aparece entre las 12 y 72 horas después del traumatismo. Dado que no existe un tratamiento directo eficaz, la prevención es fundamental. En la actualidad nuevos tratamientos se están investigando como los inhibidores del sistema renina- angiotensina, que son prometedores. En el escenario traumatológico, debe llevarse a cabo la estabilización precoz de la fractura, salvo en un grupo de pacientes que pueden beneficiarse del protocolo de control de daños y realizar la estabilización diferida. Se han desarrollado estrategias que pueden disminuir la carga de embolia grasa como el fresado a bajas revoluciones y lento, así como el sistema RIA. Los corticoides no se recomiendan en la profilaxis.
